A two-year-old girl and her family pray for her missing grandmother and great-grandmother where they once lived in Ishinomaki, Miyagi prefecture. It is one month since Japan was hit by a devastating earthquake and tsunami, and the official death toll stands at more than 13,000. Nearly 14,000 people are still missing.

These Japanese police in Ishinomaki joined the rest of the country on Monday in observing a minute of silence in memory of the disaster’s victims. Shortly afterwards survivors’ nerves were further jangled when another powerful quake hit the north-east of the country, triggering a brief tsunami warning. There were no immediate reports of major damage or injuries.

Meanwhile, Japan said it would widen the evacuation zone around the crippled power station over the coming month because of radiation concerns. Here senior officials of the plant’s operator, Tokyo Electric Power Co, bow in front of media after talks with a government minister.

押し寄せる患者 救急搬入一晩30人も 郡山の総合南東北病院

救急救命室で急患を処置する医師ら。患者の急増で救急救命室も病室として使われている=5日、郡山市の総合南東北病院福島県郡山市の総合南東北病院の救急救命室に、数人の患者が「入院」している。一般病棟に空きベッドがないためだ。
震災によって郡山市では、二つの総合病院が機能を停止した。2病院の患者計約630人を、総合南東北病院など市内の大規模病院が手分けして引き受けた。
3月の救急外来は1863人を数え、前年同月比で4割増えた。救急搬入661人は5割以上の増加だ。院長の寺西寧さん(57)は「一晩で30人を救急搬入した日もあった。そんなことは、今までなかった」と説明する。
約450あるベッドは常に満床。定員4人の病室に6人を収容したりしているが、もはやスペースに余裕はない状態だ。
寺西さんは「浜通りの人が来ている影響もある」と言う。福島第1原発の事故後、郡山市には原発周辺から最大で約5000人が身を寄せた。
病院にあった放射線量測定器で患者を測ると、高めの数値が出た。「どうしたらいいのか」。初めての事態に、何のマニュアルもない病院職員は戸惑った。
「精神的に不安定になって郡山市を離れた職員もいる」と寺西さんは打ち明ける。残った職員で放射線などの基本を「にわか勉強」したという。
寝たきり患者に十分な人員を割けないのも悩みの種だ。急性期病院に慢性的な症状の患者をケアできるスタッフはほとんどいない。床ずれができたり、肺炎をこじらせたりする患者が増えてきた。
療養病床のある病院に空きができれば転院をあっせんし、空いた病床に新たな患者を受け入れる。そんな毎日が続く。
原発事故の影響で浜通りの医療体制は崩れ去った。その影響は中通りにも及んだ。今後、避難地域が拡大しないとも限らない。寺西さんは「事故が収束しないことには、医療も元には戻らない」とみる。
「(福島第1原発から見ると)郡山市の背後に大きな病院は少ない。万が一、ここから患者を移すような事態が発生したときは、福島全県の医療が崩壊する時だ」
寺西さんは「最後のとりで」を守る覚悟を固めている。
